Գիտնականները պարզել են, որ գեների չնչին մուտացիաներն էապես ազդում են քաղցկեղային բջիջների վարքագծի և իմունային համակարգի արձագանքի վրա։ Հետազոտության արդյունքները հրապարակվել են Cancer Discovery գիտական ամսագրում:
Խոսքն, այսպես կոչված, սուբկլոնալ մուտացիաների՝ նույն ուռուցքի ներսում բջիջների առանձին խմբերի ԴՆԹ-ի փոքր փոփոխությունների մասին է։ Նման գենետիկական տատանումները հանդիպում են ոչ թե բոլոր քաղցկեղային բջիջներում, այլ միայն առանձին ենթապոպուլյացիաներում: Գիտնականները եզրակացրել են, որ հենց այդ փոքր փոփոխությունները կարող են որոշել, թե որքան ագրեսիվ է դրսևորվում քաղցկեղը, որքան արագ է այն աճում, նաև՝ ինչպես է ուռուցքը փոխազդում իմունային համակարգի հետ:
Հետազոտողները վերլուծել են քաղցկեղի տարբեր տեսակներ, այդ թվում՝ թոքերի, կրծքագեղձի և աղիքների քաղցկեղ ունեցող պացիենտների ուռուցքների հարյուրավոր նմուշների գենետիկական տվյալները։ Նրանք պարզել են, որ նույն ուռուցքի ներսում մուտացիաները կարող են այնքան տարբերվել, որ բջիջները իմունային համակարգի հետ «հաղորդակցվում են» տարբեր կերպ. բջիջների որոշ խմբեր «մաքրում են» իմունային գրոհի հետքերը, իսկ մյուսներն ավելի նկատելի են դառնում արյան սպիտակ գնդիկների համար:
Ինչպես ցույց է տվել հետազոտությունը, նման սուբկլոնալ մուտացիաներն ազդում են ուռուցքների երկու հիմնական առանձնահատկությունների վրա՝ քաղցկեղային բջիջների աճի տեմպերի և իմունային հսկողությունից խուսափելու ուռուցքի ունակության վրա: Արդյունքում՝ նույն ախտորոշմամբ երկու ուռուցք կարող են բոլորովին տարբեր կերպ դրսևորվել՝ կախված այդ մուտացիաների կառուցվածքից:
Գիտնականներն ընդգծում են, որ սուբկլոնալ մուտացիաների դերի ըմբռնումը կարող է օգնել բուժման ավելի ճշգրիտ մեթոդների մշակմանը: Մասնավորապես, նման փոփոխությունների վերլուծությունը կարող է հիմք դառնալ թերապիայի անհատականացված ռազմավարությունների համար, որոնք հաշվի են առնում ոչ միայն ուռուցքի ընդհանուր գենետիկական պրոֆիլը, այլև դրա ներքին անհամասեռությունը: Սա հատկապես կարևոր է իմունոթերապիայի համատեքստում, երբ պացիենտի իմունային համակարգը նախապատրաստվում է քաղցկեղային բջիջների ճանաչման և ոչնչացման համար:
Մասնագետները նշում են, որ ուռուցքաբանության ապագան յուրաքանչյուր ուռուցքի գենետիկական պատկերի խորը մշակման մեջ է, ներառյալ գենետիկական այն չնչին տատանումները, որոնք այսօր հեշտ է բաց թողնել ստանդարտ թեստերի ժամանակ: Այս գիտելիքը կարող է բանալի դառնալ քաղցկեղի ագրեսիվ և թերապիայի նկատմամբ կայուն ձևերի դեմ ավելի արդյունավետ պայքարի համար:
