Կլինիկաները երբեմն չեն նկատում այնպիսի դեղամիջոցների տարածված կողմնակի ազդեցությունները, ինչպիսին գաբապենտինն է, որը հաճախ նշանակում են նյարդային ցավի դեպքում։ Սա կարող է հանգեցնել լրացուցիչ դեղերի նշանակմանը, որոնք նոր կողմնակի ազդեցություններ են առաջացնում. երևույթ, որը հայտնի է որպես «նշանակումների կասկադ»: Այն հատկապես վտանգավոր է տարեց հիվանդների համար:
Գաբապենտինոիդները, որոնց թվին են դասվում գաբապենտինը (Neurontin) և պրեգաբալինը (Lyrica), կարող են ոտքերի այտուց առաջացնել: Բժիշկները երբեմն սա շփոթում են սրտային անբավարարության հետ և նշանակում են միզամուղներ, որոնք ինքնին կարող են երիկամների խնդիրներ, գլխապտույտ և վայր ընկնելու դեպքեր առաջացնել:
Հետազոտողները վերլուծել են 120 տարեցների բժշկական քարտերը (մեծ մասը՝ տղամարդիկ), որոնք երկար ժամանակ ընդունել են հինգ և ավելի դեղամիջոց: Բոլորն ընդունել են գաբապենտինոիդներ, ինչից հետո բժիշկները նրանց նշանակել են կանթային միզամուղներ՝ հեղուկի հնարավոր կուտակումը բուժելու համար, ինչը ենթադրաբար կապված է եղել սրտային անբավարարության հետ: Հետազոտության արդյունքները հրապարակվել են JAMA Network Open ամսագրում:
Դեղերի ընդունումից հետո հիվանդների մոտ զարգացել են ոտքերի կամ ոտնաթաթերի այտուցներ, սակայն միայն չորս բժիշկ է կասկածել, որ պատճառը գաբապենտինոիդներն են: 69 բժիշկ դիտարկել է այլ ախտորոշումներ, այդ թվում՝ սրտային անբավարարություն և երակային կանգ, որի դեպքում արյան խանգարված հոսքը կարող է ճնշում գործադրել երակների վրա: Միայն մեկ բժիշկ է չեղարկել դեղը: Գրեթե յուրաքանչյուր հիվանդ անցել է լրացուցիչ հետազոտություններ՝ վտանգավոր հիվանդությունները բացառելու համար, և բոլորին նշանակել են կանթային միզամուղներ:
Երկու ամիս անց 28 հիվանդի մոտ ի հայտ են եկել նոր դեղերի կողմնակի ազդեցություններ՝ երիկամների աշխատանքի վատթարացում, գլխապտույտ, տեսողության հետ կապված խնդիրներ և նատրիումի կամ կալիումի մակարդակի խանգարումներ: Վեց հոգի հոսպիտալացվել է կամ տեղափոխվել անհետաձգելի օգնության բաժանմունք:
Ըստ հետազոտող Մեթյու Գրոուդոնի, նման դեպքերում բժիշկները պետք է դիտարկեն ավելի փոքր դեղաչափեր նշանակելու կամ դեղերից հրաժարվելու հնարավորությունը՝ նշանակումների կասկադի և այլ կողմնակի ազդեցությունների ռիսկը նվազեցնելու համար:
