Ամերիկացի բժիշկներն աշխարհում առաջին անգամ վիրահատել են չծնված երեխայի ուղեղը

Ամերիկայի Միացյալ Նահանգներում վիրաբույժները պատմության մեջ առաջին անգամ կատարել են չծնված երեխայի ուղեղի վիրահատություն մոր արգանդում։
Բոստոնի մանկական հիվանդանոցի բժիշկները կատարել են երեխայի գլխուղեղի զարգացման հազվադեպ արատի բուժման էմբոլիզացիա, գրել է Science Alert-ը։
Դեռ չծնված փոքրիկ Դարեն Քոուլմանը անոթային անոմալիա ուներ, Գալենի երակի՝ գանգի ամենամեծ արյունատար անոթներից մեկի զարգացման արատ, որը սպառնում էր մահացու բարդություններով ծնվելուց հետո։
Գալենի երակի մալֆորմացիան հանդիպում է 60 հազար փոքրիկներից մեկի մոտ։ Խոսքը հազվադեպ բնածին հիվանդության մասին է, երբ արյունատար համակարգում զարկերակների եւ երակների միջեւ առաջ են գալիս շունտեր, որոնց պատճառով արյունը բարձր ճնշմամբ ուղիղ նետվում է դեպի երակներ՝ շրջանցելով ուղեղի նյութի մազանոթների հունը։
Ինքնատիպ միջամտության շնորհիվ երեխան վաղաժամ է ծնվել, բայց առանց բարդությունների։
«Ուրախ եմ տեղեկացնել, որ վեց շաբաթական երեխան լավ է զարգանում եւ դեղեր չի ընդունում։ Ուղեղի վրա բացասական ազդեցության ոչ մի նշան չկա»,- ասել է Բոստոնի մանկական հիվանդանոցի եւ Հարվարդի բժշկական դպրոցի նեյրոինտերվենցիոն ճառագայթաբան Դարեն Օրբախը։
Ներկայում Օրբախը եւ նրա գործընկերները կլինիկական հետազոտություն են կատարում, որպեսզի գնահատեն մինչեւ ծնվելն այդ վիճակի բուժման հնարավորությունը։ Առաջին պացիենտը 34 շաբաթական պտուղը դարձավ։ Բժիշկներն օգտագործել են ուլտրաձայնը էմբոլիզացիա իրականացնելու համար։ Միջամտությունը հանգեցրել է պտղաթաղանթների վաղաժամ խզման, բայց երբ երեխան լույս աշխարհ է եկել, նրա սիրտ-անոթային համակարգն աշխատել է նորմալ։
Երեխայի մայրը՝ Քենիատա Քոուլմանը, պատմել է, որ իր չորրորդ հղիությունն ընթացել է նորմալ, բայց հղիության 30-րդ շաբաթում բժիշկները տեղեկացրել են, որ երեխան ուղեղի հետ կապված եւ սրտի մեծացման խնդիր ունի։
Երեխայի ծնողներն իմացել են, որ Բոստոնի մանկական եւ Բրիգամի կանանց հիվանդանոցներում կատարվում են կլինիկական փորձեր, եւ կարելի է փորձել կատարել դեռեւս չծնված երեխայի վիրահատություն։
Քանի որ ծննդաբերությունը վաղաժամ էր, երեխան մի քանի շաբաթ գտնվել է ինտենսիվ թերապիայի բաժանմունքում, որոնց ընթացքում բժիշկները շարունակել են հետեւել նրա ուղեղի վիճակին։ Աղջիկը լրացուցիչ աջակցության կամ վիրաբուժական միջամտության կարիք չի ունեցել։ Բժիշկները նյարդաբանական խախտումների, հեղուկի կուտակման կամ արնահոսության որեւէ նշան չեն տեսել, ուստի փոքրիկին մոր հետ միասին դուրս են գրել եւ ուղարկել տուն։
«Քանի որ նա ընթացիկ կլինիկական հետազոտության առաջին պացիենտն է, մեթոդը դեռ պատրաստ չէ լայն կիրառման համար։ Մեկ հաջող դեպքը բավական չէ հաջողության օրինաչափություն գրանցելու համար։ Բուժման ապագա դեպքերը կարող են այնքան էլ լավ չընթանալ, ուստի միջամտության բոլոր ռիսկերը դեռ պետք է պարզվեն»,- ասում են բժիշկները։
