Կրծքի վերականգնման վիրահատություն. ո՞ր դեպքերում են վիրաբույժները նախընտրում չանել այն

Երբեմն կրծքագեղձի քաղցկեղի բուժման շրջանակում մաստէկտոմիա է կատարվում` կրծքագեղձի հեռացման վիրաբուժական միջամտություն: Նախկինում այս վիրահատությունը կանանց մոտ մի շարք հոգեբանական խնդիրներ էր առաջացնում, սակայն այսօր այդ խնդիրներից մասամբ հաջողվում է խուսափել մեկ այլ վիրահատության` կրծքի վերականգնման շնորհիվ, որը թույլ է տալիս վերականգնել կրծքի ձեւը եւ արտաքին տեսքը:

Ինչպես NEWS.am Medicine-ի հետ զրույցում պարզաբանեց Cleveland Clinic-ի պլաստիկ վիրաբույժ Ռաֆֆի Գուրունլուօղլուն, այսօր կրծքի վերականգման վիրահատությունների մեծ մասը կատարվում է մաստէկտոմիայի ժամանակ կամ դրանից կարճ ժամանակ անց:

Տարբերակում են կրծքի վերականգնման մի քանի տեսակներ` պլաստիկ վիրաբույժը կարող է կրծքի անհրաժեշտ ձեւը ստեղծել արհեստական իմպլանտ օգտագործելու միջոցով, կարող է օգտագործել մարմնի այլ հատվածից վերցրած հուսվածքի կտոր (աուտոլոգիկ վերականգնում) կամ անել թե´ մեկը, թե´ մյուսը:

Կրծքի վերականգնման վիրահատությունն ինքնին միանգամայն անվտանգ է համարվում, եւ ինչպես բազմաթիվ հետազոտություններն են ցույց տվել, չի գերազանցում կրծքագեղձի քաղցկեղի ռեցիդիվի վտանգից: Սակայն դեպքեր են պատահում, երբ վիրաբույժները նախընտրում են այս վիրահատությունը չկատարել.

  • Եթե հիվանդն արդեն մետաստազներ ունի: Հարցը միայն այն չէ, որ նման հիվանդների ողջ մնալու հավանականությունն ավելի ցածր է, քան նրանց, ում մոտ քաղցկեղն ավելի վաղ է հայտնաբերվել, այլ նաեւ այն է, որ նման հիվանդներն ունեն նաեւ ուրիշ, առավել առաջնահերթ հոգսեր: Ճիշտ է, ինչպես նշեց բժիշկ Ռաֆֆի Գուրունլուօղլուն, որոշ դեպքերում կրծքի վերականգնում է կատարվում նաեւ մետաստազներ ունեցող հիվանդներին, եթե վիրահատությունն անհրաժեշտ է հիվանդի հոգեբանական վիճակի բարելավման համար:
  • Եթե հիվանդը ներկա պահին ծխում է: Ինչպես պարզաբանեց բժիշկը, ծխելը մեծացնում է ցանկացած վիրահատությունից հետո բարդությունների առաջացման հավանականությունը, այդ թվում` նաեւ կրծքի վերականգնումից հետո: «Իմ պրակտիկայում, եթե հիվանդը ծխում է, ես նախընտրում են նրան չվիրահատել, քանի որ նրա մոտ բարդությունների հավանականությունը մեծ է, իսկ հետվիրահատական վերքերը դանդաղ կապաքինվեն»,- խոստովանեց բժիշկը: Իհարկե, սա չի նշանակում, որ ծխող կանայք միանշանակ հետվիրահատական խնդիրներ կունենան, սակայն բարդությունների հավանականությունը նրանց մոտ ավելի բարձր է, քան չծխողների: Բժշկական գրականության համաձայն`  ցանկալի է, որ հիվանդ կանայք ծխելուց հրաժարվեն վիրահատությունից 69 շաբաթ առաջ:
  • Եթե հիվանդը ճարպակալում, անվերահսկելի դիաբետ, անվերահսկելի հիպերտոնիա կամ այլ հիվանդություններ ունի: Այս հիվանդությունների առկայությամբ, ինչպես նախորդ դեպքում, հետվիրահատական բարդությունների հավանականությունը բավականին բարձր է:
  • Եթե հիվանդը պետք է ճառագայթային թերապիա անցնի: Ճառագայթային թերապիան կարող է իմպլանտները ու փոխպատվաստված հյուսվածքները վնասել եւ վատթարացնել վերականգնված կրծքի արտաքին տեսքը, ուստի այն դեպքերում, երբ հիվանդը պետք է նման թերապիա ստանա, սովորաբար խորհուրդ են տալիս վերականգնումը հետաձգել մինչեւ այն ավարտելը:
