Թթվային ռեֆլյուքս
Հարթաբջիջ քաղցկեղ կարող է առաջանալ կերակրափողի ցանկացած հատվածում, բայց ադենոկարցինոմաները միշտ ներքևում են առաջանում, որտեղ կերակրափողը միանում է ստամոքսին: Չարորակ գոյացության ստույգ պատճառը դեռ պարզված չէ, բայց ուսումնասիրությունները ցույց տվեցին, որ գաստրոէզոֆագեալ ռեֆլյուքսի վատացման դեպքում դիտվում է ադենոկարցինոմայի առաջացման ռիսկի աճ:
Կերակրափողում մետապլաստիկ փոփոխությունների վիճակը կոչվում է Բարետի կերակրափող՝ գաստրոէզոֆագեալ ռեֆլյուքսային հիվանդության ծանր ձև: Եթե փոփոխությունները շարունակվում են, մետապլազիան դառնում է դիսպլազիա, որը հիմնականում նախաքաղցկեղային է, ինչը հանգեցնում է լիարժեք դիսպլազիայի: Վերջին քայլը ադենոկարցինոման է:
Բարետի կերակրափողով ադենոկարցինոմայի ռիսկը մոտ 10 է 100-ից: Այս չարորակ հիվանդությունն ավելի հաճախ հանդիպում է տղամարդկանց շրջանում, քան կանանց, ռիսկն աճում է տարիքի հետ: Կերակրափողի քաղցկեղով հիվանդների մեծ մասը 60 տարեկանից մեծ է:
Քաղցկեղի ամենատարածված ախտանիշը դիսֆագիան է՝ դժվարությամբ կուլ տալը: Սկզբում կոշտ ուտելիքներ կուլ տալը ցավոտ է դառնում՝ կարծես ինչ-որ բան կոկորդին մնացած լինի: Ավելի ուշ նույնիսկ հեղուկներ կուլ տալն է դժվարանում: Քաշի կորուստը, ստամոքսի կամ կրծքավանդակի ստորին հատվածի բութ ցավը ևս մեկ նշան են: Խռպոտ ձայնը, քրոնիկական հազը կամ ոսկորների ցավը կարող են խոսել չարորակ ուռուցքի տարածման մասին:
