Քաղցկեղի որոշ տեսակներ գործնականում անհնար է կանխել, պարզել են ամերիկացի գիտնականները: Դրանք առաջացնող փոխակերպումները նվազագույն չափով են կախված միջավայրի գործոններից եւ ավելի մեծ չափով՝ ցողունային բջիջների կիսվելիս ԴՆԹ-ի պատճենման սխալներից:
Քաղցկեղը փոխակերպումների աստիճանական կուտակման հետեւանք է, որոնք նպաստում են բջիջների անվերահսկելի բազմացմանը եւ ուռուցքների առաջացմանը: Այս փոխակերպումներն առաջացնում ե՛ւ արտաքին, ե՛ւ ներքին գործոնները: Արտաքին գործոններն են՝ ծխելը, սխալ սնվելը ուլտրամանուշակագույն ճառագայթների ազդեցությունը, նստակյաց կենսակերպը: Արտաքին գործոնների ներգործությունը կարելի է մեղմել, եւ հենց դա է համարվում քաղցկեղի կանխարգելում: Սակայն փոխակերպումները կարող են ժառանգաբար փոխանցվել: Դրանից զատ, դրանք առաջանում են բջիջների կիսվելու ժամանակ ԴՆԹ-ում պատճենման սխալների պատճառով:
Ամեն անգամ, երբ ցողունային բջիջը կիսվում է, ԴՆԹ-ում տեղի է ունենում գրեթե 3 փոխակերպում: Բնականաբար, փոփոխությունները վաղ թե ուշ ընդգրկում են քաղցկեղային հիվանդությունների հետ կապված գեները: Բջիջը որքան հաճախ է կիսվում, այնքան վտանգը մեծ է: Գիտնականները կարող են գնահատել տարբեր օրգաններում չարորակ ուռուցքի առաջացման համեմատական վտանգը: Քաղցկեղի առաջացման մեջ փոխակերպումների 3 աղբյուրներից (շրջակա միջավայր, ժառանգականություն եւ բջիջների կիսվելը) յուրաքանչյուրի ավանդը որոշելը ավելի բարդ խնդիր է:
Քաղցկեղով տառապող որոշ հիվանդների մոտ հիվանդության զարգացման համար բավական է ժառանգականությունը եւ բջիջների կիսվելը, ուրիշ հիվանդների մոտ որոշ փոխակերպումներ ժառանգաբար են փոխանցվել (որոնք կարելի է անվանել H-փոխակերպումներ), մյուսները՝ էնդոծին են, առաջացել են ԴՆԹ-ի պատճենման ժամանակ (R-փոխակերպումներ), իսկ երրորդները արտաքին գործոնների հետեւանք են (E-փոխակերպումներ): Այս ամենը սաստիկ բարդացնում է ընդհանուր պատկերը, սակայն հետազոտողներն, այնուամենայնիվ, գտել են բոլոր երեք աղբյուրների ավանդը գնահատելու եղանակը: Նրանց ենթադրությամբ՝ R-փոխակերպումները հավասարապես են տեղաբաշխված տվյալ տաիքի բոլոր մարդկանց շրջանում: Մինչդեռ ժառանգական եւ շրջակա միջավայրի գործոնները կարող են լայնորեն տատանվել եւ յուրաքանչյուր մարդու վրա տարբեր կերպ ազդել:
